May I ask why you wear them?
Not who you asked, but I wear heels because I'm a tall trans woman and I want to be towering and instantly noticable when I walk into any space. Also they make a nice sound when I walk

Oh man, I can practically feel the years of heels that formed those toes. It hurts.
Rheumatoid arthritis can cause toes to curl into claws. There's another autoimmune disorder that does this too, but I can't remember which. Lupus maybe? Which would be fitting by name alone
