Generally a young a beautiful free spirit (usually a woman) with an eccentric and quirky personality who "saves" the protagonist of a story by being fun and interesting and showing them life is worth living. Think Holly Go Lightly from Breakfast at Tiffany's.
I think being romantically or sexually available is an important part of the trope: that's what takes the character from being an interesting and experienced mentor, to a weird and creepy fantasy for the writer.

MPDG is wish-fulfillment for the loser male protagonist. MPDG's are one-dimensional and quirky in just such a way to make the messy, aimless, socially inept male lead somehow attractive to them. The male is possibly on the spectrum (more likely just an asshole), the MPDG is void of real personality.
A male protagonist dating a MPDG is like him dating his imaginary friend.

The "manic" in Manic Pixie Dream Girl is not a diagnosis, it means energetic. Just like "pixie" isn't implying she's fae or something.

They're both technically correct, manic means both mania and the energy associated with mania.
